2026-2027 OHSAA Physical Form Pages 1 and 2 (Health History) Spanish
Updating Student Information for the 2026-2027 School Year in Final Forms is Open. Students will be “rolled over” for the 2026-2027 school year on June 1st. Now is the Time to update your child’s Final Forms and choose their Sports/Activities if applicable for the 2026-2027 school year. If your student is planning on participating in a Sport/Activity in 26-27 you will want to complete this ASAP so you can begin receiving up to date information from coaches and directors regarding the upcoming 26-27 seasons.
Physical Helpful Hints:
1) Most Important-Please update your child’s 2026-2027 information on Final Forms. Once you have completed this step, you will then be able to print the prepopulated 26-27 physical form to take to your upcoming physical appointment with your medical doctor, practitioner, Little Clinic, Urgent Care, etc..Completing This step will save you so time in the long run and make the physical submission process to the district very easy.
2) Once you have updated all of the Health History information for the upcoming school year in your child’s Final Forms account your child’s physical will be ready to print. Please print the required physical from Final Forms before you go to the doctor. Pages 4 and 5 are the only paperwork that will be submitted to the Athletic Department. Physicals can be turned in as follows : (Submitting Physicals Electronically is Preferred)
Option 1-Upload directly into your child’s Final Forms account. (Best Option)
Scan and Save the Physical to your Personal Device (ex..Michael Black Physical 2026)
Log on to Final Forms and Look at the My Students Page
Under Actions look for folder button (middle button)-Your child’s documents should pop up
Choose File and Find where you downloaded the physical on your personal device
Click the File and Press Open
Once completed an email will be sent to the athletic department to review the physical and update your child’s expiration date-if you don’t see an update within 72 hours on a physical you downloaded please reach out to mmuuncy@miamisburg.k12.oh.us and Zach Myers at zmyers@miamisburg.k112.oh.us to look into and resolve.
Option 2-Scan and e-mail the physical to Matt Muncy at mmuncy@miamisburg.k12.oh.us and Zach Myers at zmyers@miamisburg.k12.oh.us (2nd Best Option)
Option 3-Turn in a copy of the physical to a Coach, MHS Athletic Office (black mailbox outside the office door), or MMS Main Office (Last Resort Option)
3) When registering students, it is very important to note that school email addresses must be used unless you are a homeschooled student. The district has gone to Final Forms academically. Student emails have been added for all student communication.
4) If you are a homeschooled student you may use a personal email. Please send your personal email to mmuncy@miamisburg.k12.oh.us and zmyers@miamisburg.k12.oh.us to be added for student communication.
5) Please do not create multiple accounts for the same child. If you have forgotten your username or password please use the forgot username or password feature to retrieve your child’s account information. Parents please advise your child to log on and sign their required documents on Final Forms once you have completed yours.
6) When registering your child: Click to Switch to 2026-2027 Registration. Please update/choose all of the sports/activities your child will or could participate in for the entire 2026-2027 school year prior to filling out their forms. If you do not choose a sport/activity for the 2026-2027 school year they will not be added to rosters and all of the forms that need to be signed for that particular sport/activity will not populate.

Physical Opportunities 2026-2027
Physicals: (Good for 13 months)
Option 1- make an appointment with your family doctor, Little Clinic, Urgent, Care, etc…prior to your physical expiration date.

-Physicals will be offered from Kettering Health Network at MHS on Monday, June 1st. -Roughly 40-50 students in each time slot at 6:00, 6:30, 7:00 and 7:20P.M. (if needed) Appointments will be reserved through timestamps on google sheets. Must pre-register to attend. Once a time slot is filled up, it will be removed. -Cost-$25.00-Cash or Checks made payable to Miamisburg Athletic Boosters the night of the physical, or Venmo Miamisburg Athletic Boosters @viking-boosters-1 Put your child’s name and Burg physicals in ‘What’s it For? ‘ -Parents need not be present during the physical, but are welcome to join if they would like. -Student-Athletes must come with their printed and signed 5 page physical from Final Forms. The first 3 pages with their up to date Health History and last 2 pages blank for doctors to fill out. –
